چکیده

perform bag mask ventilation on a daily basis, prior to laryngeal mask airway (LMA) insertion and tracheal intubation? It is a common criticism that (over)use of the LMA means trainees perform less intubations and I welcome the recommendation that trainees should be preferentially attached to lists where facemask anaesthesia and intubations occur. Trainees moving between theatres will also increase the number of intubations performed, but it is important not to forget the issue of extubation, another core skill for all anaesthetists and as important as tracheal intubation.

Systolic time intervals vs invasive predictors of fluid responsiveness after coronary artery bypass surgery.

OBJECTIVES Haemodynamic parameters for predicting fluid responsiveness in intensive care patients are invasive, technically challenging or not universally applicable. We compared the initial systolic time interval (ISTI), a non-invasive measure of the time interval between the electrical and mechanical activities of the heart measured by impedance cardiography, with invasively measured haemodyn...

Non-invasive stroke volume measurement and passive leg raising predict volume responsiveness in medical ICU patients: an observational cohort study

INTRODUCTION The assessment of volume responsiveness and the decision to administer a fluid bolus is a common dilemma facing physicians caring for critically ill patients. Static markers of cardiac preload are poor predictors of volume responsiveness, and dynamic markers are often limited by the presence of spontaneous respirations or cardiac arrhythmias. Passive leg raising (PLR) represents an...

Ability of stroke volume variation measured by oesophageal Doppler monitoring to predict fluid responsiveness during surgery.

BACKGROUND The objective of this study was to test whether non-invasive assessment of respiratory stroke volume variation (ΔrespSV) by oesophageal Doppler monitoring (ODM) can predict fluid responsiveness during surgery in a mixed population. The predictive value of ΔrespSV was evaluated using a grey zone approach. METHODS Ninety patients monitored using ODM who required i.v. fluids to expand...

Prediction of fluid responsiveness using a non-invasive cardiac output monitor in children undergoing cardiac surgery.

BACKGROUND This study evaluated the ability of a non-invasive cardiac output monitoring device (NICOM) to predict fluid responsiveness in paediatric patients undergoing cardiac surgery. METHODS Children aged <5 yr undergoing congenital heart surgery were included. Once the sternum had been closed after repair of the congenital heart defect, 10 ml kg(-1) colloid solution was administered for v...

Pulse Oximetry Wave Variation as a Noninvasive Tool to Assess Volume Status in Cardiac Surgery

OBJECTIVE To compare variations of plethysmographic wave amplitude (DeltaPpleth) and to determine the percent difference between inspiratory and expiratory pulse pressure (DeltaPp) cutoff values for volume responsiveness in a homogenous population of postoperative cardiac surgery patients. INTRODUCTION Intra-thoracic pressure variations interfere with stroke volume variation. Pulse pressure v...
